Glenn van AsSpectatorHi folks………sometimes I just start shooting photos and lo and behold something turns out of nothing.
THis lady came in and saw the associate who told her she needed a simple patch to her tooth til she could afford a crown.
She waited for me (2 months) and then when I saw her I had an hour and thought……..oh oh.
The DL cusp was broken on the lower right 2nd molar and tough to see (pt. cant open wide at all).
I first used the argon to find out where the tooth structure was and found it to be at the height of the osseous level.
I then prepared with the erbium laser with water to remove tissue tags at 30 Hz and 80 mj or so and thought all was ok til I removed the rest of the amalgam and then the last picture on the first page (pic #8) shows the darn ML cusp just came off as it was cracked as well (this always happens on those darn lower molars).
Now I got some fun…………
I didnt know what to do …..buildup, endo plus bu, pin retained laser etched core buildup and temp crown is what I decided to do after osseous recontouring with the laser on the lingual at 30 Hz and 120 mj with 600 micron tip to get 3mm of tooth .
I then placed 2 tiny pins and explained to patient that it may lead to endo.
Then we built up the tooth in resin , reprepped the tooth and placed a stainless steel crown on the tooth which was long to push the tissue back.
I went 30 mins overtime here and it was tough. I was over lunch time and didnt want to ask my awesome dental assistant to get the impression material.
We placed a temp and will re evaluate as to whether we need endo or not in 4 weeks.
Tissue will heal by then.
Kinda a cool case I thought……….

I’m not being critical at all but just want to offer some some thoughts on my experience with core buildups:
1) Contrary to my training, I placed my last pin in the Fall of 1989. With modern dentin bonding agents, particulary the new self etch primers combined with Erbium laser treatment, I bond my cores with absolutely no fear of them becoming dislodged during prep, impression or removal of provisional crown.
2) In past years I placed many cores with a composite which was radiolucent. Some were long-term treatment plans and I would take radiographs of these patients which had acrylic provisional crowns/bridges. When I viewed these radiographs, I couldn’t differentiate the core from the tooth and couldn’t tell if I had left caries.
Also, probably because I wasn’t using magnification, through long term recalls I found that I had placed some final crowns on radiolucent buildup margins. Difficult to tell whether recurrent caries at the margin or just a radiolucent composite buildup so I stopped using radiolucent core materials and now only use radiopaque light cured posterior composites.

Glenn van AsSpectatorHi Allen………I noticed that myself that it was radiolucent on the final PA but was bound and determined to show it anyway for my mistake.
I have had cores come out in the past (usually when taking of the temp) but never with pins placed. Having said that I am still in the minority but slowly letting go as I try my laser in more and more spots with holes and troughs drilled into dentin as mechanical retention.
No problem with the constructive criticisms………..thats why I post these things. Remember for me it is easy to shoot photos through the scope. Takes no energy or time at all.

Just got back from Europe, what a great trip, and experience. They are doing nice things with Erbium there. Nice case too. I too, like Al, stopped using pins a long time ago. I notice that the tooth seems to crack radially during placement. Do you see that with the scope? I am wondering. Nice service though. Lasers are great!

Glenn van AsSpectatorHi Mark…….I am off to Hinman. I too am considering removing pins and I do far less than I did before but the bond strength of the restorations to dentin is questionable even with lasers so…………
Anyways will post one or two cases before I go.
You getting the information you need from the companies for scopes. If you need any information from Global that you arent getting……..let me know.
