    Hi folks here is a case I did yesterday that was compromised by finances.

    I did the man a favour as he has lost his job and his wife and times are a little tough financially.

    He had decay under the crown on the labial and what we did was put the laser 400 micron tip on and remove tissue at 20 Hz and 30 mj and no water with topical only.

    I placed a silk cord in the tissue and that acts as a marker.

    To clean out the decay, 30 Hz and 100 mj and water. No anesthetic needed as the tooth had endo.

    NO ETCH ON THE PORCELAIN at all and the resin is point 4 opaque and I silanated the porcelain.

    I am off to Hinman so wont be able to respond.

    PS I have to do the central in a while ……..he wanted to do the worst out of the two teeth so I know that the other one will need doing soon.

    Nice treatment and welcome back from Atlanta(100 miles north of my hometown)!!!

    Did you see any white sparking when you lasered the porcelain? I know Mark Colonna has posted some photos where it did appear the Waterlase cut feldspathic porcelain. He reported “sparking” so I’m afraid to try this with mine.

    Again, Glenn, nice service. This is a GREAT example of why a microscope is so essential when doing this type of case. I am not so sure that with 6x I could be so exacting with my laser. Clap, clap, clap as you would say to others. Thanks for teaching us as usual!

    The neatest part was the laser welding Mark………that was something I had never tried and it was fun to see that it does work. Warmed up the tissue and it melded together………..

    I know that some will just shake their head. I would too if I hadnt done it.

    Amazing.

    Glenn,

    Tissue welding works. Well establish principle of warming the blood to create hperthermia, not vaporizing it. Takes place when the blood is warmed to 50 degrees C.

    (pssssst, that’s what the long pulse duration are used for when you can’t simply defocus in the deeper pockets and you want tissue welding down deep=hemostasis)

    Glenn, Cool case. Please don’t take this wrong! One of the cosmetic problems with this type of case is the difference in the translucency between the porc and the composite. Most of the time it is not a problem as the lip will hide it.
    Try a base layer of something opaque like A3 opaque Kolor +Plus from Kerr. Make a nice translucent composite look like and opaque porc.

    PS I have heard of this for occlusal accesses in previously crowned teeth.
