Here’s a case of a recurring furcal inflammation after an endo which was treated by a specialist. Second lower left molar is an abutment to a 4-unit PFM bridge. Endo done about 1 year or so ago and the patient has been on a 3-month hygiene schedule to keep furcation area inflammation under control. It never quite improved and recently took a turn for the worse.
An exploratory flap was done using the erbium to make the incision and the 980nm diode to clean out the granulation tissue. Quite a significant breakdown on the mesial aspect of the distal root. Beyond MTA repair I imagine. The patient recalled that the specialist had trouble keeping the canal dry….
Anyhow, a sad conclusion to an overtaxed and heavily treated molar. It will likely be replaced by an implant in the near future. An implant will first be done at the pontic area while the extraction site heals with bone grafting. I will post the progress as treatment commences.
