    40 yo male presents for crown preparation tooth #30. Profound local anesthesia establised lidocaine/articaine, gross reduction with diamond in electric handpiece, extensive caries excavated with Er:YAG(ConBio DELight) and composite core placed.

    An 810 nm diode laser(ConBio DioDent) was used for gingival retraction around the crown margin. A 400 um bare, initiated fiber applied in a contact, hot tip mode at 1.0 Watt and Continuous Wave for approximately 45 seconds.

    I was impressed by the clean, bloodless field. Is it better to NOT rinse the treatment area after application of the diode for retraction? Rinsed with water before thinking(duh!!) but did not get any hemorrhage.

    Hi Al,

    Very nice photo documentation, but especially your descripton and detail of what you used when.

    It doesn’t matter about rinsing following activated tip with diode when the tissue is as healthy as the tissue was in this case. When the tissue is more edematous (larger capillary diameter–say 500-600 microns), THEN rinsing can cause “breakthrough” bleeding.

    I always risnse to get the dried blood “bits” and tissue “crumbs” outta there so my impression is clean–yours is immaculate!

    Hi Al,
    Excellent margin. I think technicians should pay you for making their life easier and more enjoyable .
    Did you replace the composite before crown prep?
    Would you consider replacing the MO on the 2nd molar to achieve better contour and contact point. Also to avoid replacing it in the near future.

    Yes that is a new composite buildup, the old composite was bombed underneath and yes the MO amalgam on #31 looks like crap and I should have placed a new composite.

    Amazing how pictures give you a different perspective isn’t it? Dr. Glenn knows a thing or two.

    Just back from Anaheim lecturing for a week and havent really had time to do much else yet, so I havent posted in a while.

    Try some H2O2 for any tissue tags you get as there are times when it is rough with a little tissue here and there. I put it in an Ultradent brush that you would typically use for Astringident.

    Really nice impression and no fear of bleeding or fear of nicking the cementum with the laser.

    Allen, I was up early this morning and reading your post. I have been putting of using my Lasersmile for doing this troughing around preps. You got me off my butt today. Way cool. I used to use hydrocolloid and electrosurged a ton. I can still hear a lecture by Dr Spears about troughing around preps. He would always get a laugh when he called his elctro sx unit his laser. No sense in spending big bucs if you can do the same thing with electro sx. After using the diode laser today on my C&B cases all I can say is Spears must have never tried it.
    Using the diode laser was much nicer. The tissue looked great . I felt I had less tissue tags, better control and better hemostatus.

    No, the retraction on the case I showed you was done with the DELight Er:YAG. Thanks for the comments. Now we need the Diagnodent laser, I think we got the cart before the horse.

    Thanks, yeah I was pleased with the healing at one week. Patient said there was no discomfort.
