    Hi All,

    David, your case on the lateral is beautiful. What did you do for the other lateral that was missing?

    Glenn, like Jeff, I used AA in short spurts routninely not just on ablation margin but on pulpal floor as well, with high evacuation to capture a fair bit of the 27u Aluminum Oxide powder. Little danger to the scope as the barrier is protecting it from the debris.

    A few photos to show removal of the ablated margin as per attached case.

    Someone asked me at a course whether erbium will perform bleaching. I jokingly said it will indeed. It will give you a white smile. A frosty white one that is!

    I don’t have my Waveleng…sorry, The Journal of Laser Dentistry in front of me but I did notice there is an abstract in the back that discusses bond strengths with er:YAG Vs. etched and non-etched enamal. Did anyone else see that? It was not a very detailed article.

    Andrew nice case.
    Glenn, I know what you mean about lab veneer , chairside veneeer and Miguels fee. I belive there was a time factor with tis patient that Ruled out a lab veneer.
    I hate doing chairside veneers. I am never happy with them and I am always happy with the labs.
    You had to ask about the other lateral!! This is one big compromised case as you will see from the x-rays. The kid is 18 Y/O.

    Wow David…………..what a neat case. I have someone like this who has two primary teeth that are both going to come out.
    I was going to do a little 2 tooth partial , but does this last for a while.

    Is there any longevity to this……..as this kid is off to college on the other side of the country and the two teeth that are loose and coming out are the lateral and canine on the upper left.

    What do you think…..will this work for a 2 tooth span?

    I read all of Mike Melkers beautiful post on DT, it was gorgeous and I like his split dam.

    How does he clamp it …..on the premolars.  THANKS GLENN-NO ANESTHETIC ON THE CASE. I CLAMP OVER THE RD WITH #0 CLAMPS AND THE PT WAS COMFORTABLE. SPLIT DAM. I DRY THE VESTIBULE AND TUCK EITHER THE DAM OR A COTTON ROLL WITH THE DAM TUCKED UNDER. THE DRY TISSUE HOLD SIT WELL.

    Wow , I noticed Mikes fees ……….1400 for two teeth.  That is what I almost get for two crowns (around 1500).

    MY FEES FOR THIS TYPE OF CASE RANGE FROM 238-700/ TOOTH. MY INDIRECTS GO FROM 965-1350. THE RESTORATION IS THE PATIENTS CHOICE. TO ME THIS IS MINIMALLY INVASIVE DENTISTRY.

    I am not sure if that is justifiable for the longevity of the case.

    IF THE CASE IS RESTORED IN PORCELAIN AND CHIPS, WE START OVER. IF THE COMPOSITE CHIPS, IT CAN BE REPAIRED. THE LONGEVITY IS VERY DEPENDANT ON THE PERSPECTIVE OF LONGEVITY AND REPAIRABILITY AND THE PROGRESSIVE NATURE OF RESTORATIVE DENTISTRY.

    The dentistry is beautiful and I learnt alot.

    Glenn, Thanks. Which two teeth. depending on the patient and the case, this tx can last for years.
    I am not so sure about using sticktech. I had a long talk with the guy from Ribbond. The new ribbond maybe a better choice because of its ablity to inhibit crack formation. Then again Sticktech is pretty sweet stuff. Really easy to handle. I find ribbond a bit tougher to deal with. Though I just used the new material and it is far better then the old ribbond.

    Mike: I am glad that you posted here. Your dentistry is absolutely beautiful and I think your photography is awesome. I think it is wonderful that you take the time to show us your work because you are gifted.

    Thanks for the hints about split dam……..it looks great and I should do that more often and I think I will start trying to do more. It looks alot better in the documentation.

    Its easy for me to take pics because I can just go click click (actually my assistant does) on the scope.

    Its a piece of cake , but you take tons of photos on your cases and its not so easy.

    I admire you for that. I also want to commend you for going to 6.0 loupes….. good for you.

    In any event, with respect to your fees, dont get me wrong you deserve it and the work is great. You are very good with direct resins. I hope that one day I can get up to your level.

    As for DT, I love Howard and the crew there, but I do get disappointed with some of the hacking that goes on there. There are very few with enough Kahonas to post their cases but alot of members in the peanut gallery.

    The same folks making the same anti technology rants gets a little stale for the folks who like lasers as well.

    I marvel at the professionalism here and it seems that if someone steps out of line that he/she is quickly brought into line by being told that this board is quite professional in its manner, and that the primary role is to educate laser enthusiasts or those thinking about lasers, not to debate ad nauseum if there is a role for lasers in private practice.

    If you peruse through the posts there are an amazing number of threads on everything and remarkably there is acceptance of a wide variety of wavelengths, brands and techniques.

    Really that is unique and something I commend Ron for. Its really a pleasure to post here because people ask intelligent questions, thank you and act in a civil fashion.

    Thanks for posting and dont be a stranger, its great to have first class clinicians on here and your photos and work speaks volumes for the quality of your dentistry………

    This is a cool site. We are starting a local laser users group tonight and this will be a great resource. I am new to lasers (6 months) but am having fun. Hopefully I can make it over here more often.

    Maybe you will even talk me into one of those schmansey scope things. biggrin.gif

    The scope helps with ergonomics, documentation and visual acuity which helps create precision in your dentistry.

    Nothing outstanding but just so easy to take pics of.
