    I will soon be investing in a laser system. I am taking several courses soon. Several questions re: insurance coding come to mind. I can see many uses for soft tissue laser–especially in C and B. If you are doing a crown lengthening proceedure and it is only soft tissue removal, do you code D4210, gingivectomy? If the proceedure is bony, do you code D4249, clinical crown lengthening–hard tissue? If so, the code description states “…requires reflection of a flap…”. How is this handled with laser tx? I have seen several examples of a “closed” technique with interproximal osseous recontouring for crown lengthening purposes (no flap reflected). Your thoughts on coding and laser treatments would be appreciated. Thanks, Dan

    Dan: I looked at your post and you have several valid concerns and it is something which unfortunately I am not the right person in Canada to give you the answers.

    There are somethings that can be billed out with the laser, and in addition there are some things that cant be ( like gingival curettage) so one of the thing you must prepare for is that some of the cost of the procedures will be saddled onto the patient.

    SOme dentists even charge a Laser usage fee to patients to offset the cost of the laser. Whether you surcharge is something that only you can answer and it depends on what role the laser is going to have in your office.

    If you are planning to buy a Delight laser contact me as there is a marketing guru for laser dentistry…Dr. Don Wilson who has used his laser to build huge numbers through specific marketing approach.

    Thanks Glen, I have appreciated following your cases on this and other sites–very nice! I have not seen any threads regarding this topic and have started to wonder if maybe this is a “landmine” type of issue that some may not want to address.     Hi Dan……..I will be presenting in Las Vegas in Feb and doing some live stuff through the scope there so we will get the chance to meet.

    I bought the laser to do less anesthetic type dentistry particularly on kids and it was only afterwards that I started thinking about the ROI bit.

    Not my strength I admit……….

    I do hope that others from the USA address the codes and what they do and dont bill out.

    I am sure it is State specific, where do you practice.

    I will be in Vegas in Feb also and can give you a full run down of the process I went through with my laser purchase. Of course if you have one by Feb then we can just rambleon about all it’s amazing applications. smile.gif

    Thanks to Glen and Paul. I know the insurance issue can be touchy. I am not trying to find a way around the codes–only am curious how this is handled. I am looking foraward to meeting you both in LV and will, of course, pick your brains! I practice in Washington State, near Seattle. Probably will not have laser by Feb. I am still looking for the “perfect” system–ha! Currently investigating Nd:YAG systems–I like what i am hearing about LPT and Periolase–we’ll see. Dan
