    Just an update of Consult thread

    Patient finally decided to try and save teeth but would not see periodontist for surgery or ortho for extrusions . Patient also wouldn’t wear braces for the 3rd time. 14 probed 10+ mm.

    1 week post op LPT (Periolase) of left side TX. Time for some occlusal adjustment. Found it very interesting the correlation between which teeth were in hyperocclusion and where the bone loss was most severe.

    After adjustment patient sat up and said  “WOW, that’s the first time I’ve been able to bite on both sides since I got braces off”.

    Made my day!

    We’ll see if we can get some new attachment and bone back.

    Ron,

    Based on the COF movie–I suspect the patient will need some more OA to better balance out the forces. In these cases, one can’t be afriad to cut the heck out of the teeth to get balance. (I remember how timid I was to adjust. Now I wail……)

    Did you make him an occlusal guard?

    Patient cancelled right side Tx for next week-had to go out of town on family business. More OA to come and OG once right side Tx (week after next).

    What is a TEK Scan?

    TEK Scan (link here) is a computerized oclusal analysis system. There is a sensor system that the patient bites on and the contacts are recorded as they happen. This ‘movie’ can then be replayed to aid in occlusal analysis and adjustment. The images above are just 1 frame of the ‘movie’, in this case the time where there is the maximum amount of contact. The relative force of contact is shown thru color (red the hardest). The red and white line as well as the red & white diamond show you the center of force and how it changes over time.

    Hi Ron,

    Cool stuff. A graphical measurement to locate occlusal disharmony certainly makes OA easier to treat. Great patient education too.

    Another item on the shopping list?

    OK, I confess………Tek Scan has been one of my “secret weapons”.:o

    The software runs about &#364000.  I have mine hooked up to my laptop, and bring into the operatory when needed.  Ask them about their new USB cabability.  Ron and I are using parallel ports.  They work just fine, though, but USB might be easier to use in tight quarters.  The new 4.0 software is very stable.

    Ron, can you post the bar graph image?  It will give a different perspective by showing the relative heights of the excessive forces.

    Tek Scan is NOT a necessity for getting excellent results during the occlusal adjustment portion of LPT.  But there are times when it has been extremely helpful (for patient records and documentation), and I can see it making a difference in results for some situations (molar teeth, sliding contacts to a stop).

    Yeah, the price I’m not sure of. I bought mine 5 years ago, and I may have forgotten the price. I do know I paid full retail, and I have no &#36&#36 interest in the company. I lectured for them once, but received no compensation, expense remuneration, no free stuff, no free sensors, not nothin’, which is what I insisted on.

    I called the lecture “Occlusion for Dummies” and I was the Dummy!!wow.gif

    Just a bit of follow-up.
    Pt tx last April. Disappeared for 10 months except 1st 3 month recall (&#36- this health professional decided not to participate with BCBS and it has devistated his practice, but that’s another story).
    Showed up when he popped out #7. 7& 10 removed and bonded back into place. No recall since July. bonded 7 back in place and did some probing, adjusted occlusion more in area 14/15. No mobility or bleeding anywhere. Convinced pt to set appointment for recall and to take some new xrays.

    Today patient called and canceled recall, guess the xrays will have to wait. I was pleased with the result but would like to retx 13/14/15 area.
