I have tried to treat a couple of these. The are tough and stubborn. Like you said, a lot is tied to their immune status (i.e stress).
Modification of the tissue itself–with a shallow penetrating wavelength like CO2 or erbium followed by near IR laser for biostim would be my choice for symptomatic and temporary relief.
They need to address the underlying cause of thier condition for our efforts to contribute to any long term result.

What do you think about using an Erbium for non-erosive lichen planis? Can the lesions be eradicated by the shallow effect or would a Nd:Yag be better. This is not a loaded question. With your experience I thought you would have some worthwhile thoughts.
What do you think would be the results with diode biostim alone?
I have several patients (mostly smokers) who can get the lesion to ameliorate by decreasing or eliminating smoking and it comes back as the smoking continues.
Makes me go hummmmm.
whitertthSpectatorI have used my waterlase at .25 watts without 5% air to effectively treat desquamative gingivitis…my hypothesis is that u could do the same with erosive lichen planus…. Wether we r biostimulating cells to create a change of do a form of microdermabrasion and allowing the healthy cells to reepithelialize I’m not sure but I do know this works…Good Luck…..
jetsfanSpectatorThis 8 yr old girl came in for a 6 month continuing care visit. This reddened erosive area was not present last visit. Mother says its been present for a few months. It is uncomfortable to touch. Does not bleed easily. She has no unusual habits.No fever, no ulcerations anywhere else., excellent health. Looks a little like Ron K’s desquamative gingivitis but very localized(an 8 year old though?) I am not sure what I am dealing with, any thoughts? suggestions? I did attempt to palliate with Waterlase at .25W 11% A. I will follow closely.
