Glenn van AsSpectatorHi folks: I cant claim fame to this alone, as I think that I saw Mark Colonna do a video of this when I was at the ADA but it got me to thinking what if I could use the laser to extract a primary tooth without anesthetic and today I did it.
I am pretty proud of the pics on a 6 year old squeamish but kind little girl. I dedicate this to Ron as he and I talked for the first time by phone today at length about lots of things including scopes and I told him how proud I was that he had made this site what it is and also how far he has come in his knowledge of lasers…….
ANyways, no needles , ONLY TOPICAL for this. It had a big long root and was not mobile at all , but the pictures tell the whole story.
The tooth was coming in palatal to the primary tooth and with the recession that was present it got me to thinking that I bet the tooth is long still and that the maxillary central missed the palatal………
WOuldnt it be cool to extract with only topical.
First applied one touch topical by Hagar, you could use TAC or EMLA. Then used a spray by Xylonor (Sp) and finally ultracare by ultradent which comes in a syringe with a green canula that you can extrude the topical in the crevice.
I then tried to laser anesthetize with high energy and water settings at 140 mj and 30 Hz (4.2 watts) for the full 120 secs.
Next off I had the 400 micron tip in the socket working at first 30 Hz and 50 mj……to sore
then 30 Hz and 30 mj …….to sore.
finally 20Hz and 30 mj……fine.Worked for a couple of mins (2 probably) and got it loose so it was wiggling and then used a gauze to rotate.
Could you have used anesthetic……..sure and you would have been faster.
If this little girl was your daughter and you had the choice, would you try to do this without anesthetic…….
Anyways something not presented in photos I dont think and just another thing you can try with the laser.
No tears at all for the little girl and a happy happy mom.

Try out that Tricaine Blue/Topical Anesthetic Gel stuff also if you get a chance. I’m afraid to try it on kids due to the high concentration of anesthetics. We are getting great results on adults as pre-anesthesia for our injections. Yesterday performed gingival retraction around implant abutment with diode laser using only the Tricaine Blue.

Did you use 600u or 400u fibre tip?

The great thing about these cases is that they stay in the back of your mind once you see them- the power of seeing. I know somewhere down the road I’ll be seeing a patient and think to myself-‘hey this is similar to what Glenn(or Al, or Mark,or Andrew,or Kelly or fill in the name of your choice)…. did and if I add this or that to it, it would work in this case’
Speaking of the Ultradent stuff, we take one of their small syringes like you use w/ their astringent and a tip w/o the brush on it and keep EMLA in it. Seems to work pretty good for getting the EMLA into a sulcus also.
BTW Glenn, thanks for the compliment, but what makes this site work is ALL the people who participate and share. One of the many things I’m thankful for this Thanksgiving.

EMLA , Hurricance , Blue Goo…..you name it, they all can be used.

Vince………the Maple Leafs needed to gain a false sense of security and also needed to boost Pat Quinns rating as a coach. They did both at the expense of the Canucks but just watch in the playoffs……the Canucks will go far this year I think.

This is my source for “Tricaine Blue” which is compounded as “Topical Anesthetic Gel” by John Hollis:

ph: (615) 327-3234 fax: (615) 320-068It’s 10% lido, 10% prilo and 4% tetracaine so some this is some high powered stuff. I’ve seen variation in compounding from different pharmacies – what I like about Hollis’ compound is it seems to adhere quite well to dried gingiva/mucosa plus the blue color makes it easy to see where the topical was applied.
They will send you a faxed Rx for you to fill out and return. It’s about a ũ US per gram but then there are handling/shipping fees.
