Tom DaviesSpectatorWe recently had Janet Press, RDH, come to our office for training on incorporating our diode laser into our perio program. Janet has years of experience, and is extremely capable both in the science and in the practical applications of lasers in perio. She has attended every annual meeting of the American Academy of Periodontology for the past 22 years. She is on the clinical faculty and is an instructor at LVI.
Janet came to our office on a Wednesday afternoon. She gave us a lecture from 1 to 4 PM. A workbook and literature information were included. Janet is incredibly knowledgeable on this topic, especially on the growing body of medical evidence which links periodontal disease and inflammation to other areas of the body. She also provided practical information on how you incorporate this into your office.
The next day was a clinical day. Janet instructed myself and my hygienist on micro-ultrasonic instrumentation and the use of the diode laser for perio therapy on patients we had scheduled. She is unbelieveably skilled clinically, and she really helped us in a hands-on way as to how to perform the therapy.

dkimmelSpectatorI think we all like to hype having a great experience. Too often we take a course and find it to be a waste of time or come away with one helpful hint.
Hopefully Tom will drop back in and let us know the rest of the story.
CE has become expensive in not just cost but time out of the office. That is one of the great things about this forum. We all share and learn from each other.
Tom DaviesSpectatorTo answer some of the questions about my original post:
1. I think Janet’s protocol is mainly aimed at decontamination; however, de-epithelialization is occurring concurrently. Elimination of the bio-film in the perio pocket is the ultimate goal.
2. We have yet to see any patients back for 6 week therapy/re-evaluations, so I cannot report on any hard results yet.
3. This is being done by the hygienist, so there is no flap elevation. First visit you tackle the worst pockets or areas showing the most signs of disease. Microultrasonics, irrigation, diode laser. This may not necessarily be a quadrant; it could be two molars in two different quads. Next appointmnet, go to the next area or quad, treat that area, and re-lase the worst pockets from appt. #1. Continue until all teeth treated, then have 6 week therapy, re-evaluation.Janet’s training was great, and the cynic who posted the blast is a foul ball.

It is very encouraging to hear about a hygienist who is into lasers. I had an unhappy experience with one I employed part time. She was very good, and patients loved her, but set in her ways. Refused to learn anything about lasers. Even turned in her resume the day after I insisted she attend a local laser seminar put on by a leader in the dental laser field.
