I went to Dentin Bonding Agents as my pulp cap in 1995 (Kanca & or Bertolotti methods), acid etch, then DBA, then Flowable compsite, then bonded composite restoration. Threw away the Calcium Hydroxide.
Since 1999, Nd:YAG laser to remove the remainder of caries when see pink, if exposure cauterize w/ Nd:YAG, then Etch,DBAs, Flowable.
Now Nd:YAG, Self-etching DBA (I-Bond or One-Up bond), Flowable.
4-6 months later some fail, I blame on carious exposures, with CaOH techniques most would fail, many within days.
Not a reason to buy a Nd:YAG, but another use for the tool if you have one.

Marks instruments are awesome. I purchased my first set and will use his kit soon for all restorative composite resins……..why?

1. His spoons are wonderful especially for erbium procedures. What I find is that the dentin gets soft (especially the carious stuff) with all the water spray and it will peel out very nicely and easily with the spoons at high mag.

2. His pluggers come in smaller sizes and are wonderful for use in resins. Not sticky and the right shapes.

3. Mark has some nice placement instruments including burnishers for the resin and in addition he has some beautiful felts that are so thin.

I havent really got alot of composite instruments over the years and really really like his tools that he designed.

He worked hard on them and they really deserve a look.

Buy a set , try them, you wont be sorry, I know I was astonished and I mean that.

Now for the poster boy, I think you have done more to try and educate users than most Mark and I meant the comment in a caring way.

I have a couple of cool cases I am gonna show later tonight between writing articles for dentistry today and for oral health here in Canada……..sheesh.

Finally, pulpal encounters ( of the worst kind) are something I think that is much more predicatable with magnification. I know that I accidentally hit the pulp far far less now with the scope since I can see the blush etc.

I still dont have the success some do with the laser sterilization but have tried it sometimes and it works sometimes.

Great posts as ususal and I would love to see a laser meeting (no specific laser companies) just lectures on how to improve technique……….what do you think Ron……

David: I am so glad that you say that you need 4.5X to see what you are doing. Now you know why I love working at 10X power with the scope with the laser and why I feel naked without it when I go on the road

Try the filters if you can get them inside the loupes as the diode can be very very bad for your eye.

The ebium affects your cornea and your lens and the diode can affect your retina.

Dont risk it , honestly, get the filters for the loupes. Your eyes will thank you!!

WIth the ConBio delight we put the tip inside the sheath and autoclave them in their separate bag.

Be cautious to not roll instruments on them if you place the tips only in the bag.

We place the tip INSIDE the sheath loose and that prevents accidental breakage.

In my 12 years of third world dentistry I have extracted one tooth but have done many restorative procedures.  I hire one or two Bolivian dentists to take out teeth as needed.  They volunteer and I pay them &#362.50 per tooth.  I just refuse to take teeth out and work on children as much as possible and others who will benefit from restorative procedures.  Over time we have been working to develop a school based fluoride to restorative programs.  This year we took care of some wonderful deaf mute children from a special school.  What a privilege.  I did have to do it the old fashioned way with the high speed handpiece.
