    Hi Allen: I have been using a metal matrix for all my isolation either a cut tofflemire strip like this one I did today, or an interguard from Ultradent which I have shown before. It protects the adjacent tooth and seems to reflect the laser energy and give you faster cuts on the tooth.

    In this case I used a 400 micron tip with the Delight laser. A cut tofflemire was used in this case for isolation.
    I used a rubber dam with topical (one touch by Hagar) and didnt use any anesthetic except that.

    Settings were 30Hz and 100-120 Mj which was quite high for the enamel and then I lowered it to 70 mj for the decay. Used a tiny bur for the last little bit.

    Patient rated pain as 0 out of 100. In addition patient has alot of attrition. I used marks nice felt type instrument which is very thin and I like it alot for sculpting the composite.

    I finished the case with burs and polishing cups.

    I have a question I am not sure of. Maybe someone can answer it. Is there a handpiece manufacturer, that warranties their handpieces more than one year after you buy them? If so, what company? I just was wondering?

    Hi David,

    No need for flaps to de-epithelialize with pulsed Nd:YAGs and diodes.  The perio and CO2 folks need to flap to gain access for their “free beam” lasers.  It is really passe.

    Check out the annimation on http://www.millenniumdental.com to see fiber-optically de-epithelialization at work with a pulsed Nd:YAG.

    Lee,
    I anticipated that the patient wouldd react to the water and air and began at some distance probable beyond a defocused range and then gradually came closer as I watched the patient’s reaction until I was just far enough away not to frost the tooth. I had warned the patient that the water might bother a little so the patient worked with me to tolerate a little sensitivity. I kept testing the senstivity by zooming in a little closer until I could frost the surface at 1.5w 11/7 using a 6mm G tip. The frost was for cementing purposes. The whole process went rather quickly 30 to 45 seconds I would say. Thanks for asking.

    Thanks for the feedback Ron and Glenn. Thanks to Glenn now we now know it’s probably safe to use our Erbiums around metal composite matrices.

    That’s a cool case Glenn! I like the way you extended your prep onto the worn incisal edge. Ron’s forum and the way we learn from one another has been “Priceless” for myself!!!!

    Thanks for the article!

    No need to tone down what you are saying, especially when you are right.

    I don’t believe for a second that water is doing the cutting with an Er:YAG or Er,Cr:YSGG.

    Turn the water and air off an Er,Cr:YSGG, like I did in Phoenix last Friday. SOMETHING sure as hell cut that extracted tooth! And it wasn’t water! Maybe it was……..no I’m not gonna go there, I promised Ray.

    Bob,
    I enjoyed the animation.  Can you share a little about the clinical progress one might expect to see in a typically responsive case of chronic periodontitis with loss of attachment and pockets exceeding 5 millimeters.
    Over what time period and clinician time committment involved.

    I believe that a radio ad was run in the SF area in a coop marketing type strategy. I don’t know what the results were. Talk to Keith, he should be able to help.

    You better believe I “believed” you because you have a reputation as a dentist with a vast knowledge of laser applications in dentistry and if this test didnt work out I was gonna be calling you for reimbursement on a trunk fiber!!!!!!!

    just curious,
    do you guys an gals use the kavo polisher or prophy jet or some other particulate stain remover( other than pumice or prophy paste) before taking readings. I find that the maintenece, after each use, of Kavo’s unit is so cumbersome that my hygienists do not want to use it.

    Anyone tried removal of tori with the laser. If you can do crown lenghtening seems do able!
